Uh, the attachment came out weird, and I am not quite sure what the poll is aimed to accomplish?
Are the poll choices the questions you wish to ask?

Could you just type out your birth details here?
And let us know if it is solar adjusted or not.
If not, tell us your birth place as well.
Then we can chart it out ourselves for better confirmation.
